Team-based care in the community
CST is an intensive, community-based service for eligible adults with complex mental-health or co-occurring substance-use needs. A multidisciplinary team helps the person stabilize, strengthen daily functioning and connect treatment with housing, medical and community supports.

Care is individualized. Your assessment and treatment plan determine the goals, frequency and services that are appropriate for you.

Most CST work occurs face to face in the home and community, with office or telehealth contacts used only when clinically appropriate and allowed. The team maintains regular contact based on the person-centered plan and coverage requirements.
CST requires a qualifying diagnosis, functional need, comprehensive assessment, service order and health-plan authorization. Service availability depends on county, contracted network and current team capacity.

We collect the clinical history, recent discharges, current providers and coverage information.
The team confirms whether CST criteria appear to be met and whether another service is more appropriate.
After approval, the team develops a person-centered plan and establishes the contact schedule.
