NC Medicaid 1915(i) Services

1915(i) services help eligible NC Medicaid members build the skills and supports needed to live in their own homes and communities. TSGBH coordinates with the member, care manager and health plan to deliver authorized supports available through our program.

How This Service Can Help

Care is individualized. Your assessment and treatment plan determine the goals, frequency and services that are appropriate for you.

Individual & Transitional Support

One-to-one skill building for routines, community living, self-management, relationships, appointments and greater independence.

Transition Support

Help preparing for changes such as moving from a facility, treatment setting or unstable living situation into a community setting.

Coordinated Planning

Work with the member’s care manager, family and approved providers so services follow the authorized care or support plan.

Supports May Address

  • Daily routines, home-management and community-living skills
  • Using transportation, appointments, benefits and community resources
  • Communication, self-advocacy and relationship skills
  • Preparing for a move or maintaining stability in a community setting

Important Program Limits

  • An independent assessment is required before 1915(i) eligibility is established
  • The health plan determines which services and hours are authorized
  • TSGBH can provide only services for which it is enrolled, contracted and staffed
  • 1915(i) services do not replace emergency, inpatient or medically necessary clinical care

Service Delivery

Services are delivered according to the approved care plan or Individual Support Plan, usually in the home and community. Goals, frequency and activities must connect directly to the authorized need.

Eligibility, Coverage & Scheduling

1915(i) eligibility and access depend on NC Medicaid enrollment, qualifying clinical and functional needs, independent assessment, plan authorization, provider network and current capacity. Intake will coordinate with the assigned care manager.
