How We Create Impact

Our work focuses on access, continuity, dignity and practical results rather than isolated appointments.

Access to Care

Statewide telehealth for appropriate outpatient services and community-based programs in approved service areas.

Whole-Person Coordination

Connection among therapy, substance-use treatment, primary care, medication services and social supports.

Community Stability

Support for people navigating hospitalization, justice involvement, housing disruption, family stress or other barriers to sustained care.

What We Measure

  • Referral response and successful connection to the right level of care
  • Timely assessments, service starts and required documentation
  • Engagement, attendance and progress toward person-centered goals
  • Care coordination, service completion, satisfaction and safety

How We Work With Partners

  • Health plans, hospitals, primary-care practices and specialists
  • Schools, courts, probation, reentry and community organizations
  • Housing, employment, faith, nonprofit and social-service partners
  • Families, caregivers and people seeking services directly

Responsible Reporting

We do not publish unsupported outcome numbers or copied claims. Public impact information is released only when the underlying data, period, population and measurement method can be verified.

Community Partnerships

Partnerships should improve access, reduce duplication and create a clear handoff for the person receiving care. TSGBH reviews proposed collaborations for privacy, clinical, compliance and operational fit before launch.
