Practical support for community living
Supported Living helps eligible individuals build and maintain the routines, skills and community connections needed for greater stability and independence. Support is individualized and delivered according to the approved service plan.

Care is individualized. Your assessment and treatment plan determine the goals, frequency and services that are appropriate for you.

Strengthen decision-making, communication, boundaries, problem solving and asking for appropriate support.
Services take place in the home and community on a schedule established by the person-centered plan. Support should build skills and independence rather than completing every task for the individual.
Eligibility, funding and authorization vary by program. Intake must review the person’s current plan, care manager, diagnosis, functional need, requested hours and service area before confirming availability.
