What kind of care do you need?

Choose one option below. Each button sends your request to the correct TSGBH team.

Need both medical and behavioral health care? That is okay. Start with the care you need now. With your permission and as allowed by law, our teams can coordinate your care.

Option 1

I need therapy or behavioral health services

Choose this for therapy, psychiatric medication management, child and family services, substance-use treatment, CST, Peer Support, Intensive In-Home, 1915(i), DWI services, Supported Living, Life Coaching or reentry support.

Psychiatric medication management is part of this pathway.

Option 2

I need a medical appointment

Choose this for virtual primary care, non-emergency sick visits, blood-pressure or chronic-condition follow-up, general medication review and appropriate refills, wellness support, laboratory coordination or medical referrals.

Use this for general medical care, not psychiatric medication management.

Integrated care, in plain language: You do not have to choose only one type of care. When you receive both services and authorize coordination, your medical and behavioral health teams can work from a more complete picture of your needs.

The form you choose goes directly to the team responsible for that type of care.

The appropriate team checks coverage, location, clinical needs and current availability.

We explain missing information, assessment requirements and the next available scheduling step.
