Stronger care starts with listening to the community.

Leadership from TSG Behavioral Health & Community Services recently visited our Elizabethtown, North Carolina office to meet with community partners serving Bladen and Robeson counties and better understand where gaps in care remain.

Our team met with Department of Social Services (DSS), court partners, and other community stakeholders to discuss the challenges individuals and families face when trying to access and remain connected to care.

Closing the Gaps Between Systems

For many people, behavioral health needs do not exist in isolation. A person may also be navigating substance use concerns, physical health conditions, housing instability, transportation barriers, court involvement, family needs, or difficulty coordinating services across multiple agencies.

When those systems do not communicate effectively, people can miss appointments, experience interruptions in treatment, or become disconnected from services altogether.

Our conversations focused on how local partners can work together to improve access, continuity of care, referral coordination, follow-up, and connections to community-based resources.

Our Integrated Care Approach

TSG’s integrated care model looks beyond a diagnosis and focuses on the whole person. That means strengthening connections among behavioral health, substance use treatment, physical health, social supports, DSS, the courts, families, and community resources.

This visit was not simply about telling the community what TSG offers. It was about listening.

Our community partners see challenges firsthand. Their perspective helps us understand where referrals stall, where individuals become disconnected, and where better collaboration can strengthen the local continuum of care.

As TSG continues serving southeastern North Carolina, our goal is not growth for growth’s sake. Our goal is to help build a more connected, responsive, and accountable system of care so individuals and families can receive the right service, at the right time, in the right setting.
