Intensive In-Home Services (IIH)
IIH is a team-based service for eligible children and adolescents with serious emotional or behavioral needs who are at risk of out-of-home placement or need intensive help to remain stable at home and in the community.
How This Service Can Help
Care is individualized. Your assessment and treatment plan determine the goals, frequency and services that are appropriate for you.
Crisis Planning
Develop a clear plan for warning signs, de-escalation, after-hours resources and when emergency care is needed.
School & Community Coordination
Coordinate with schools, medical providers, care managers, courts and other supports when authorized.
IIH May Be Appropriate When
- A youth has serious emotional or behavioral symptoms that significantly affect daily functioning
- Outpatient therapy alone has not been enough
- There is risk of hospitalization, residential placement, removal from home or repeated crisis involvement
- Caregivers can participate actively in an intensive home-based plan
What Families Can Expect
- Frequent contacts in the home and community
- A multidisciplinary team and a clearly defined person-centered plan
- Caregiver coaching, youth skill building and coordination with other providers
- Regular review of safety, progress and readiness to step down to less intensive care
Service Area & Availability
IIH is delivered primarily face to face in the home and community. It is available only in approved service areas where TSGBH has an enrolled and staffed team. Intake will confirm current county coverage.
Eligibility, Coverage & Scheduling
IIH requires a comprehensive clinical assessment, qualifying diagnosis and functional need, service order and health-plan authorization. The health plan determines eligibility and approved duration. Referral does not guarantee admission.
