Virtual Primary Care

TSG Health & Wellness provides telehealth-only medical care for appropriate everyday illnesses, follow-ups, wellness support and medication questions. The clinician may recommend laboratory testing, imaging, an in-person examination or emergency care when virtual care is not sufficient.

How Referrals Work

One referral is enough. Intake will route the request and explain any additional records, assessment or authorization needed.

Submit a Referral

Provide the person’s contact information, location, coverage and the main reason for seeking help.

Clinical & Coverage Review

We review safety, medical necessity, service criteria, payer requirements and current program capacity.

Assessment & Care Plan

A qualified professional identifies the appropriate level of care and coordinates the next step.

Referral does not guarantee admission. Final eligibility depends on assessment, diagnosis, functional need, authorization, service area, provider network and capacity.
